    Below are frequently asked questions from individuals regarding our low-cost dental plans.     Can I see any dentist I choose with CarePlus?
    You’re free to choose any dentist you like at any of the 15 Dental Associates locations in Wisconsin.

    I have a toothache right now. If I enroll in CarePlus will this be considered a pre-existing condition?
    No, CarePlus has NO Pre-Existing Condition clauses and NO Waiting Periods. Your toothache can be treated as soon as possible and will be covered by your CarePlus insurance plan.

    Who is eligible to purchase the CarePlus Individual plans?
    Depending on our dental care plan – Dental Associates Diamond or Platinum – the requirements may vary. For example, eligibility depends on whether you have other dental insurance coverage, if you are covered by a PPO dental insurance plan or if you need dependent coverage.     How do I enroll in CarePlus dental insurance?
    The most convenient way to enroll is by starting the enrollment process on this website. Complete the Enroll in CarePlus, then pay online or contact one of our friendly team members to finalize your enrollment. You may also purchase a policy at any Dental Associates facility when you come for an appointment. Finally, you can call CarePlus at 1-800-318-7007 to request an application form.

    When does my dental insurance policy become effective?
    Since CarePlus has no waiting period, your policy becomes effective almost immediately. You will be covered after the following three simple steps are taken:

    1. You complete the form(s) required by CarePlus
    2. You pay your annual CarePlus premium
    3. CarePlus approves your application


    Can I add a dependent to my existing policy at any time throughout the contract year?
    If you were enrolled in a single plan you would terminate that plan, receive a prorated refund, then enroll in a family plan and pay the family premium. See the dental contract for specific rules regarding changes in membership status.

    Is there a limit to the number of family members that can be covered by a family plan?
    There is no limit to the number of dependents that can be covered by the Diamond or Platinum family dental plans. The plans cover your spouse or qualifying domestic partner and any number of dependents up to the age of 26.

    What is the 10-day Right-to-Return contract?
    You may return the contract within 10 days after receipt to CarePlus. If you do so, the contract is void and all payments made under it shall be refunded.

    How can I cancel my policy?
    After the initial 10-day period, you can terminate your policy by sending written notice to CarePlus Dental Plans at any time.

    How long are CarePlus premium rates guaranteed?
    CarePlus will provide a 60-day notification of any rate change.

    What methods of payment are accepted for payment of CarePlus premiums?
    We accept cash, check, Visa, MasterCard, Discover or American Express for payment of premiums.

    Can I pay my premium online?
    Yes! You can pay your premium online after submitting your enrollment application.

    Who do I contact when I have questions?

    What if my child needs to see an orthodontist for braces? Do I need to get referred?
    No, you do not need to be referred to another dental office. Dental Associates provides all specialty care – including orthodontics, pediatric dentistry, periodontics, oral surgery, cosmetic dentistry, dental implants, endodontics and major restorative services – and has specialists on staff to service your needs.

    Is teeth whitening covered by insurance?
    Yes, teeth whitening is covered by all of our individual dental care policies.

    Are there specific dental services not covered under the individual policies?

    Does CarePlus offer coverage for Medical Assistance/Medicaid (T-19/BadgerCare/Benchmark) patients?
    Dental Associates no longer accepts new Medical Assistance/Medicaid (T-19/BadgerCare/Benchmark) patients. Since all care is provided by Dental Associates, CarePlus doesn’t have a plan compatible with Medical Assistance/Medicaid (T-19/BadgerCare/Benchmark).
